When Mod creators say, "create a CLEAN save," does that mean starting the game from scratch?

Stupid question alert. But over on Nexus I've seen this written several times. Or with variants of the word clean (fresh, etc).

Does that mean start the game over?

But that was just one of dozens I've seen. I'm new to the scene and just want to make sure I'm on the same page.
